Please read the waiver below and sign electronically. This covers your child(ren) for the duration of their participation in any Power Play PE program.
In consideration of my child(ren) participating in programs offered by Power Play PE LLC, held at Innovative Strength & Conditioning (315 & 317 12th St, Sacramento, CA 95814), facilities owned by Singleton Legacy LLC, I the undersigned parent or legal guardian agree to the following:
I understand that participation in physical activity involves inherent risks, including but not limited to physical injury. I voluntarily assume all such risks on behalf of my child(ren) named in this form.
I release, waive, and discharge the following from any and all liability, claims, or damages arising from my child's participation or presence at the facility, whether caused by negligence or otherwise:
I grant permission for emergency medical treatment to be sought for my child(ren) if I cannot be reached. I confirm that my child(ren) are in good physical health and able to participate in physical activity.
I understand this waiver covers all Power Play PE programs and remains in effect for the duration of my child(ren)'s participation.
By signing this waiver, I consent to photos and videos being taken of my child during Power Play PE sessions and used on the program's website and social media. To opt out, email PowerPlayStaff@gmail.com at any time.
