One waiver covers all your kids. List every child who will participate in any Power Play PE program — group PE, personal training, or Little Movers — in the form below. If you add another child later, just email us and we'll update your file.

Photo and video policy: By enrolling in Power Play PE, you consent to photos and videos being taken during sessions and used on our website and social media to share the joy of what we do. If you prefer your child not be photographed or filmed, simply email us at PowerPlayStaff@gmail.com and we will make a note on your family's file. No explanation needed.
Questions About the Waiver
Do I need to sign a new waiver every year?
Only if our waiver language changes, which we'll let you know about in advance. Otherwise your waiver stays on file for as long as your family participates in Power Play PE programs.
Is an electronic signature legally valid?
Yes. Electronic signatures are legally binding under the federal ESIGN Act and California law. Your signed waiver is stored securely with a timestamp and submission record.
What if I have more than three children?
List as many as the form allows and then email us at PowerPlayStaff@gmail.com with any additional children's names and dates of birth. We'll add them to your file manually.
My child has a medical condition I want to tell you about. Where do I do that?
There is a field in the form for medical conditions, allergies, or anything else you want Coach Tracey to know before your child's first session. Please be as detailed as you'd like. That information stays private and is only used to make sure we take great care of your kid.
I don't want my child photographed or filmed. What do I do?
Just email us at PowerPlayStaff@gmail.com and let us know. We'll add a note to your family's file and make sure your child is never included in any photos or videos we take. No explanation needed and it won't affect their participation in any way.
I already signed a waiver at ISC. Do I need to sign this one too?
Yes. Power Play PE is a separate program and requires its own waiver for participation. It only takes a couple of minutes and you only need to do it once.
